Please take a moment to read the following instructions on the registration process:
  		• You will be asked to provide some information which will assist the Site Administrator in registering you and your family in the ChurchDB system.
  		• Some of this information is required and indicated on the web page by an asterisk (*) and bold face print.
  		• Other information requested is optional but providing it will expedite the registration process.
  		• After you submit the information, it will be forwarded to the Site for action by administrative staff. You will receive an email confirmation with your new User ID and Password as soon as you are entered into the ChurchDB system.
  		• Indicate Number of Family Members (including yourself) that you would like to include in this registration:  (up to 6) ?
